It's a pun. The German word "blasen" as a verb means (among other things) "to give head" whereas "Blasen" (the noun) means "blisters".
So "Sie kann blasen" means "she knows how to give head" whereas "Sie kann Blasen an den Füßen nicht ertragen" means "She can't stand blisters on her feet".
Same with "Sie ist so eng" ("She is so tight") vs. "Sie ist so enge Schuhe nicht gewöhnt" ("She isn't used to such tight-fitting shoes").
Such puns are quite common in German fun songs. Another example is "Hey, ich hol' dir einen runter - einen Stern" which plays on the double entendre of "runterholen" meaning "to bring down" and "to jack someone off".
Another (non-sexual) example is the famous chorus of Rammstein's "Du hast" where the abbreviated line ("Du hasst mich") means "You hate me" whereas the full line "Du hast mich gefragt" means "You have asked me".
