The song is full of metaphors; I translate most of them literally because I couldn't find the exact metaphors in Indonesian.
*I'm not sure what that line means, I couldn't even grasp the full meaning in English. Any suggestion is welcome
**"Cetak biru" literally means blueprint. Indonesian phrase for master plan, "rencana induk" don't really suit the feel of the song for me.

"leave hope in my wake" I think means "I'll leave (my) hope behind".

What other parts did you have trouble understanding?

I wonder if the song's title is actually an idiom, because I couldn't find the exact meaning of the phrase "sometimes you're the hammer/nail". Hence I translated it literally.

Also, I'd like to know what "I'll close the distance" actually means.

It reminded me of the proverb "if all you have is a hammer, everything looks like a nail" which means either of the two:

1. With limited tools, single-minded people apply them inappropriately or indiscriminately
2. If a person is familiar with a certain, single subject, or has with them a certain, single instrument, they may have a confirmation bias to believe that it is the answer to/involved in everything.

Sometimes the person is the hammer (hurting, wounding, exposing, etc. the other) and sometimes they're the nail (they drive the nail in deeper, hurt or wound the person more).

"I'll close the distance" usually means "The length of the space between two points" or "Gradually get nearer to someone or something".

They mean something like "I'll stand up for those can't stand up for themselves, so that they're on equal ground between themselves and what they're struggling against", so that they have a chance of overcoming it.
