I’m not proficient in Portuguese but it is quite similar to Italian and mainly to Spanish, so I hope to help you, if I understand correctly the text.

In the first lines: di tuo > del tuo (the possessive requests the article in Italian – or the articulated preposition)
revirando tudo > che mi sconvolge tutta
Tô com saudade dessa cara linda: “saudade” is a Portuguese word known all over the world but difficult to translate. In Italian it could be “nostalgia”, “desiderio struggente”, but not ”brama” that means a more material desire, a lust. “Brama” is properly used in the ninth line.
Me pedindo fica só mais um segundo > che mi chiede di restare ancora un secondo
(a minute is sixty seconds in both languages)
madrugada fria > mattino freddo (sounds better in Italian keeping the same order of the Portuguese sentence)
Tô tão carente > Ne ho bisogno (ne = of it; no need to translate “tão”)
Vivo um pesadelo > vivo in un incubo
Tô com vontade de enfrentar > Voglio affrontare
Ser pra sempre > essere per sempre (without “di”)
um amor que vibra: “vibra” is good in Italian; and so also “poesia”
Numa poesia em forma de canção > in una poesia sotto forma di canzone (or less literally: in una poesia che sembra una canzone)
menos que a medade > meno della metà (less literally: meno che dimezzata)
Sou incapacidade de viver por mim > sono l’incapacità di vivere da sola
