Definite:
"If the worst comes to the worst" (you need the article in English)

Consider:
"I say nothing" – perfect English, maybe sounds better here

"Then my pupils make known they want to dilate" (I think this definitely needs changing; I'm not quite so sure of the best English equivalent)

Drugs and pills pouring doesn't sound too good. "Drugs and pills cannot calm me" perhaps

"Cardiologists have nothing to help me" pehaps
