I’m not sure here, but I would translate the title as “Thanks that you’re here, my mom”.
You have given me birth → You have given birth to me (your variant sounds a little off)
With much love and goodness you have gifted me → You have gifted much love and kindness to me (as far as I remember the direct object is what not whom, but this line is really tricky)
good → kind (through the text, good is kind of “excellent” while “kind” is kind of “gentle”)

No need for the "my" in front of "mom", it's kind of implied :D

I'd also change the first line. "You are the one for me" has a romantic connotation. Something more like "You are the only one I have" would fit better, imo.

In the former case my suggestion is “You are my sole one in the world”.

OK, I spent the better part of today asking different native speakers and the general consensus is that "sole one" doesn't work(except for one guy who thought it was a foot fetish pun). The best reason given was: "Sole" already contains "one" in its meaning.
