It's a fun song, I watched this performance on TV (they're showing parts of it Italy as part of the Sanremo Music Festival), it looked interesting as they were all dressed and covered in gray paint and I wanted to know what it meant.

A (very little) misunderstanding. I try to explain the sense, you'll be able to put it in the most suitable way in English.

=” but if there is the game/the game takes place, the commando doesn’t act/is not there”
The meaning is that the whole commando goes to the stadium to watch the game.

You really choose a very difficult text: In the last lines, the word "pizzo", doesn't mean "lace", but "protection money" (the one that criminal organisations ask as a due tax to traders and contractors)...
ma l'Italia "non ci sta"= Italy disagree/ doesn't want to play that dirty game/doesn't want to go along with them... It's the same expression you find in the context of "commando"

You are right, it is very dense. It references several accidents and scandals that marred the Italian healthcare system in those years - unsafe blood transfusions ("I say no to your plasma"), botched surgery ("If you forget the clamps") and the aforementioned "ghost physicians". Gosh, these memories coming back after 20 years make me feel so old! :)
