This is a very hard to song to understand, even in Portuguese. There's no context: some people say it's about masturbation or even about having sex with a prostitute. Zé Ramalho says he was under influence of heavy drugs when he wrote this song, so it's even harder to understand. This is NOT a literal translation, it just tries to convey the original sense of the lyrics. About the title: 'Frevo' can be understood as a confusion, some kind of agitation - in the lyrics, probably caused by a woman. That's why I went with 'Lady Confusion'.
