Author's comments:

First: I know it's in the final part not the translation of the german version, but either on youtube or on my phone 'Revolverheld' sings it the way I wrote it.
The words in the brackets are sung by a choir.

In the German version of this text on here it is written
'Bist du nicht auch längst schon müde
der Straßen, der Menschen, der Massen'
But he actually sings it that way:
'Bist du nicht auch
längst schon müde der Straßen,
der Menschen, der Massen'
So he asks whether you are already tired by all three of this and not if you are tired.

In this song he sings 'Lass uns gehen, lass uns gehen, lass uns gehen'
So he is actually telling you that he want to leave with you the place you are at right now.

He also sings 'Und träumen vom Sommer in Schweden'
which mean literally 'And dream of summer in Sweden'
He actually means that you and him dream of that.
