Go off/dive off the deep end

To jump into a problem, a crisis, an unknown or unforeseen danger without first checking out the situation, or the facts of the matter.
In a literal sense it means to jump into the deep end like in a pool, or body of water only to discover you are 'in over your head'. It implies that the action was taken without planning or concern for the effects unseen.
