Here are some suggestions:
1. Line 1 : « lần cuối » is not « last word » but « last time »
2. Line 3 : « em nói đi », « đi » here does not man « to leave ». The term « đi » is used to invite, to beg some one to do something. « em nói đi » mean « please tell me » or « come on, tell me »,
3. Line 4 : Same about « em nói đi ». Then : « you tell you’ll never be sad »
4. Line 8 : Should be :« In the next thousand lives, I will love only you » (we believe that after death, each of us will recommence another life, again and again, indefinitely, unless….but this is too long to explain here !)
5. Line 9 : « vời vợi » do not mean « beautiful », but « endless, immense » in time or space
6. Line 8 : « em nói đi » « em nói đi » !!!!!
7. Line 13 : « muôn kiếp » is the same as « muôn đời »

I made most of the changes, as they seem to make sense.
I still have a couple of questions:
The idea you mentioned of a "thousand years/lives" is in the line " Anh chôn dấu đời **ngàn năm** lạnh giá", right? And that ties in with " Anh xin muôn kiếp yêu em mà thôi," correct?

I understand 'Anh xin muôn kiếp yêu em mà thôi' as '(even if I never see you again,) I shall never love anyone else than you', that is 'There will be no other love in my life than you'. Which is only very slightly different from your own translation...
