It's a little strangely worded but I feel that I understand why they did it. I've listened to this song many times over the years, but the wording is also strange in most of their songs (mostly due to the lack of punctuation as you have mentioned in your comment).

I understood it like this:

I aim to have control (but) I stumble and fall

I reached for you but fell right through1
I'm confused and I'm losing ground
You embrace me in my mind
I hold on to my sweet beliefs.

I call out to my own reality, I scream out all my fears
I'm overwhelmed by all the insanity, I've sacrificed a million tears2
I call out in pain, my weaknesses fight to become whole
I'm overwhelmed by madness, I lost my soul in the darkness.

I woke up alone but in my heart
you were there whispering my name
I want to go back to the same old place
where we beloved, until death do us part
I close my eyes and numb my heart
I wish for sleep but I only face my truth
You embrace me in my mind
I hold on to my sweet beliefs.
